Overview
Registered Nurse – Case Management (Home Health) — Portland, OR travel assignment. Start date: 04/20/2026. Duration: 13 weeks. Weekly pay: $2,701 - $2,782 per week. Shift: Days, 8-hour shifts (40 hours per week). Setting: home healthcare – hospice. This travel role focuses on planning, coordinating, and directing home care services to meet patient needs in their homes. You will manage a patient caseload efficiently, provide independent nursing care as needed, and collaborate with other core disciplines to meet care goals. You’ll serve as a resource to patients, families, and other care team members.

Responsibilities
- Plan, coordinate, and direct home care services to meet the patient’s needs in their home environment.
- Manage a patient caseload in an effective and efficient manner.
- Provide direct patient care when appropriate, while collaborating with other core disciplines according to the patient’s plan of care.
- Serve as a resource to patients, family members, and other care team members to support care goals.
- Document assessments, interventions, and outcomes in alignment with the care plan and regulatory requirements.
- Promote patient safety, symptom management, and quality of life within a home-based care setting.

Requirements
- Active RN license (state to be determined for assignment)
- Experience in case management or home health setting preferred
- Strong assessment, communication, and care coordination skills
- Ability to manage a patient caseload and collaborate with a multidisciplinary team
- Comfort with day shift scheduling and travel to patient homes as part of the assignment
