About the Role 

The Mobile Nurse Practitioner (NP) provides comprehensive, patient-centered care to Institutional Special Needs Plan (I-SNP) members residing in skilled nursing facilities (SNFs). This role focuses on delivering proactive, high-quality care to a frail, medically complex population through routine visits, acute evaluations, and longitudinal care coordination within a managed care model. 

This is a full-time W-2 position with a defined end date of December 2026. This role is aligned to current program needs, with the potential for extension based on business requirements and performance.

Working collaboratively with an interdisciplinary care team—including Registered Nurses (RNs), Care Coordinators (CCs), facility staff, and other healthcare professionals—the NP plays a key role in improving patient outcomes, reducing avoidable hospitalizations, and enhancing quality of life while supporting effective utilization of healthcare resources. 

This is a highly collaborative, patient-facing role that requires strong clinical judgment, effective communication, and the ability to translate clinical findings into actionable care plans. The NP partners closely with Avail Healthcare operations to support efficient scheduling, timely and accurate documentation, and overall program execution, including alignment with Medicare Advantage and I-SNP requirements. 

In addition, the NP conducts Health Risk Assessments in collaboration with members and, when appropriate, their authorized representatives to identify clinical and social risk factors. Based on these assessments, the NP develops individualized care plans —including problems, goals, and interventions—that inform care coordination efforts and support the interdisciplinary team responsible for ongoing member care. 

Success in this role requires comfort working independently in the field, adaptability in a dynamic environment, and a proactive approach to continuous improvement and program development. 

Location & Work Environment  

This is a primary mobile, field-based role requiring residence within Maryland, with regional coverage for assigned area. Candidates who are centrally located within the coverage area and able to support efficient daily travel are strongly preferred. 

Standard hours are Monday through Friday, 8:00 AM – 5:00 PM EST. Remote work requires a reliable high-speed internet connection and a dedicated, HIPAA-compliant workspace to ensure the security and confidentiality of patient information and organizational communications. 

Key Responsibilities 

  • Deliver comprehensive primary and acute care services to I-SNP members residing in skilled nursing facilities, including initial, routine, and transition of care (TOC) evaluations 

  • Assess, diagnose, and manage acute illnesses, chronic conditions, and geriatric syndromes in a medically complex population 

  • Develop, implement, and adjust individualized, goal-oriented care plans in collaboration with patients, families, and the interdisciplinary team 

  • Order, interpret, and act on laboratory and diagnostic studies to guide clinical decision-making 

  • Prescribe medications and therapeutic interventions in accordance with regulatory standards and evidence-based practices 

  • Provide ongoing, proactive follow-up to monitor clinical status, evaluate treatment effectiveness, and prevent avoidable complications or hospitalizations 

  • Manage a dedicated panel of patients across custodial (long-term care) settings 

  • Oversee transitions of care, including admissions, readmissions, and discharges, ensuring continuity and quality of care 

  • Collaborate closely with nursing staff, rehabilitation services, specialists, and facility leadership to coordinate and optimize patient care 

  • Participate in interdisciplinary care plan meetings and contribute to comprehensive care planning 

  • Recognize and respond to early signs of clinical deterioration with timely intervention 

  • Provide patient, family, and caregiver education to support disease management, treatment adherence, and goals-of-care alignment 

  • Deliver palliative and supportive care as appropriate, including advanced care planning and goals-of-care discussions 

  • Maintain accurate, timely, and compliant documentation in the electronic medical record 

  • Adhere to I-SNP program requirements, quality metrics, and regulatory standards

  • Communicates clearly with patients and families to support engagement and goals-of-care alignment  

Operational Excellence 

  • Completes timely, accurate, and compliant documentation aligned with I-SNP and Medicare Advantage requirements  

  • Contributes to performance on key quality metrics, including care gap closure and risk adjustment accuracy 

Adaptability & Ownership 

  • Operates independently and efficiently in a mobile environment  

  • Identifies opportunities to improve workflows and supports ongoing program growth 

Qualifications: 

  • Master’s or Doctoral degree in Nursing (MSN or DNP) from an accredited program 

  • Current national certification as a Nurse Practitioner (FNP or Adult-Gerontology NP) 

  • Active, unrestricted Maryland Nurse Practitioner (NP) license maintained in good standing and in compliance with all applicable state-specific practice regulations throughout employment 

  • Active Maryland DEA registration with prescriptive authority, or eligibility and willingness to obtain prior to start date. 

  • Minimum of 1–3 years of clinical experience required ideally in geriatrics, long-term care, institutional settings, mobile care, or value-based care models preferred 

  • Experience managing medically complex or frail elderly populations preferred 

  • Familiarity with risk adjustment; HEDIS/STARs measure and care transitions best practices preferred.  

  • Demonstrates advanced clinical judgment with strong assessment and diagnostic capabilities, effectively managing care for complex, high-acuity patient populations. 

  • Comfort with remote work tools, EHR platforms, and technology-enabled care delivery models 

  • Strong communication, organizational, and documentation skills 

  • Ability to work effectively in a hybrid environment with evolving workflows and processes. 

  • Residence in Maryland with ability to travel within the assigned regions as needed 

  • Authorization to work in the United States 

Compensation & Benefits 

  • Base Salary: Base salary range of $135,000-$155,000, depending on experience, scope, and qualifications. 

  • Retirement:  401(k) retirement plan with a 2% employer match, subject to plan terms and eligibility requirements 

  • Performance Bonus: Employees may be eligible for a discretionary performance bonus based on individual performance, role-specific metrics, and overall company performance. Bonus structure and targets vary by role and are not guaranteed. Employees must be actively employed on the payout date to receive any bonus. 

  • Annual Compensation Review: Compensation is reviewed annually and may be adjusted based on performance, market data, and organizational factors. Adjustments are not guaranteed. 

  • Mileage Reimbursement: $0.725 per mile for travel exceeding 30 miles per day, up to $200 per day. 

  • Benefits Package: Comprehensive benefits include medical, dental, and vision coverage. The company offers a Direct Primary Care (DPC) model paired with a high-deductible health plan and covers 100% of employee medical premiums, with dependent coverage subsidized. Additional benefits include company-paid life and disability insurance, and employer contributions to a Health Savings Account (HSA). 

  • Paid Time Off: Employees accrue 15 days of paid time off annually, increasing with tenure. PTO accrues per pay period. 

  • Holidays: 8 paid company holidays plus 1 floating holiday annually.
